I acknowledge that the first two months held each calendar year are FREE and there is a $4.99/mo charge for any additional months. Perfect Tan will credit any money spent on holds back to you upon return from hold in the form of PT Ca$h (good toward products).

ie: If your payment date is March 31st that is

considered your "March payment".

I understand that Perfect Tan requires a 7 day notice, before the date of my monthly draft, to hold my account.

Please note: *ROLLOVER PASSES do not qualify for holds.
